Position Description: Penn Medicine at Home - Palliative Care - is looking for a Team Manager to oversee approximately 30 direct reports covering Pennsylvania (Bucks, Montgomery, Delaware, Chester counties) who work with approximately 240 patients. Direct reports will include nurses, physical, occupational and speech therapists, social workers, and home health aides. We are looking for a candidate who has experience in leadership with direct reports, who enjoys overseeing nursing/allied professionals and who is motivated in; elevating practice, implementing new projects and processes, working in and with leadership and also hands on, problem solving and managing multiple priorities. This is an innovative, cohesive and engaged team, that will continue to grow. Penn Managers assume 24 hour accountability for the operation of the designated home health team. This includes the provision of leadership to improve patient care, enhance professional development and achieve clinical and operational goals. Accountabilities o Support staff in development of performance and professional growth (clinical or career) to ensure interprofessional model of care; foster staff involvement in initiatives both inside and outside of their team. o Promote and help maintain efficient patient flow initiatives through facilitation of care coordination activities. Determine gaps in efficiency and work with health care teams to meet goals. oShare/disseminate research-based clinical interventions and best practices to improve patient care and achieve cost savings. Ensure rapid and effective implementation of best practices. Participate in promotion and coordination of research-based practices that foster improved methods of patient care and practices. o Represent the interest of Penn Medicine at Home in the served community; support clinical participation in community outreach and collaborate with area schools for clinical experience. oDevelop understanding of revenue and reimbursement in health care. Optimize cost performance/workload management. Develop, review, analyze, and manage budget and financial reports, as appropriate, to achieve budget targets and organization objectives. o Ensure patient, employee and internal partner satisfaction targets and measures are established and met on a periodic basis. o Performs duties in accordance with Penn Medicine and entity values, policies, and procedures o Other duties as assigned to support the unit, department, entity, and health system organization Penn Home Palliative Care services are designed for patients receiving treatment for a life-limiting condition. We support our patients' medical care by addressing their physical, psychological, emotional, spiritual and social needs. Our goal is to provide the best in individualized palliative care so patients can remain at home, preserve their independence and adapt to lifestyle changes during this challenging time. Penn Home Palliative Care is designed for patients who are still receiving treatment while hospice is most appropriate for patients who wish to focus on comfort care. Penn Home Palliative Care offers specialized services such as: Pain management Help with nausea, fatigue, shortness of breath and anxiety Help with understanding your medications Physical and occupational therapy to maximize quality of life After-hours emergency support via telephone or nurse visit Assistance in completing an advance directive or living will Support for family and caregivers Telemedicine Our Penn Home Palliative Care team includes: Your physician who remains involved in your medical care and maintains regular contact with the Penn Home Palliative Care nurse. A registered nurse who visits you at home regularly to make sure you are comfortable and as pain-free as possible. A social worker who may be available to provide additional support. Our social workers keep your friends and family informed and can offer emotional and psychosocial support. A home health aide who assists with personal care, such as feeding, bathing, dressing, and changing bed linens. Physical and occupational therapists who help you maintain your independence and adapt to physical limitations. Therapists can help increase mobility and relieve pain. Minimum Requirements: Minimum Certifications, Registry and/or Licenses Required: Currently licensed as a professional nurse or allied health professional in the Commonwealth of Pennsylvania. Degree Requirements and Minimum Experience Required: ? Minimum Bachelor's degree in Nursing or allied health care (i.e. PT/OT/SLP) and a Master's degree preferred (or enrolled in a Masters Program). ? Minimum of 5-10 years of progressive nursing or allied health care (i.e. PT/OT/SLP) experience required with demonstrated leadership and management skills.
